Required Forms for Registration

• Complete the online registration process (you should have received an email) & pay appropriate fees

• Virginia Health Form

  • All required immunizations must be up to date and certified by a Medical Provider. If your child’s annual physical exam appointment is later than September 2022, the Preschool box on the last page of the VA Health Form must be completed and signed by a Virginia Health Care Professional.

• Original Birth Certificate (for new students only)

  • Please bring the original Birth Certificate to the office for validation when registration is confirmed.

LLVP Handbook 2023-2024

• Registered Preschool families may request a hard copy of the 2023-2024 LLVP Handbook from the Preschool Office.
